At check-in, your credit card information will be collected and stored securely in compliance with PCI-DSS (Payment Card Industry Data Security Standards). No full card numbers are stored in our office, and access is restricted to authorized personnel only. This information will only be used after your insurance(s) have processed your claim and notified us of any balance due.
Once your insurance has paid its portion, you will receive a statement reflecting any remaining balance. You will have 30 days from the statement date to make payment. If payment is not received within 30 days, the balance will be charged to your credit card on file.
Please note:
PAYMENT AT TIME OF SERVICE: Payment is due in full at the time of service unless you are covered by Medicare or an insurance company with which we participate. You will be charged a $25 service fee for any returned checks, no exceptions.
INSURANCE: Patients will be asked to present their insurance card to the receptionist for copying upon check-in at the office each time they are seen for medical services. Please make it a point to bring your insurance card with you each time that you visit our office. Claims not paid within 45 days by your insurance company will become your responsibility. You will receive a statement for these services and you will need to contact your insurance company for reimbursement.
For those patients covered by insurance plans with which we ARE participating providers, all co-payments, deductibles and noncovered services are due at the time of service. We will file your insurance claim to the insurance company. In the event that your insurance coverage changes to a plan with which we ARE NOT participating providers, we will require payment in full at the time of service and we will file your claim to the insurance company as a courtesy. Any charges that are not paid by your insurance company are your responsibility. Your insurance policy is a contract between YOU and your insurance company. Any pre-certifications of procedures or testing are your responsibility. Please let us know in advance if your insurance company requires this.
COLLECTIONS: Please note, if payment is not received from either you or your insurance company within 60 days from the date of service(s), your account will be considered delinquent and subject to referral to an outside collection agency.
Patients under the age of 18 must be accompanied by a parent or legal guardian for their first appointment.
The legal guardian must present appropriate documentation verifying guardianship at the time of the visit.
After the initial appointment, the parent or legal guardian may request a Minor Consent Form, which—once completed and signed—will allow a responsible adult (age 18 or older) to accompany the minor to future appointments in their place.
Late Arrivals
If a patient calls ahead or arrives more than 10 minutes late to their scheduled appointment, the appointment will need to be rescheduled. This helps us maintain timely care for all patients and avoid extended wait times.
No-Show Appointments
Patients who do not show up for their scheduled appointment without contacting our office at least 24 hours in advance will be considered a "No-Show."

These fees will be billed directly to the patient and are not covered by insurance. All No-Show fees must be paid in full before your next appointment can be scheduled.
You will receive a text message 3 days prior to your appointment. This message will include a secure link that allows you to pre-register and complete all necessary paperwork before your visit.
In addition, you will receive an email invitation to set up your patient portal through SADIO. The portal provides convenient access to your medical records, appointment details, and more.
We encourage all patients to complete pre-registration and activate their patient portal prior to their appointment to ensure a smooth and efficient check-in process.
